When a sponsor is asking for a scholarship for a pilgrim they should first check with their mini-community if they have one and if not, they need to call, write or Lisa Snead to get a form to be filled out. (This form must be used to request a Scholarship from CCEC.) Mini-Community contact information may be found by clicking on Mini-Communities.

CCEC CLERGY TEAM APPLICATION

This is in Adobe Acrobat Reader 7.0 format.To use it, download and save it on your PC. It can be filled out on your PC, then printed, signed, and mailed to Dusty Garison (use the address on the form). Or, you may fill it out by hand (Please PRINT THE INFORMATION), SIGN IT, then mail it to Dusty Garison using the address on the form.

You cannot save data printed on the form. Fill it out, then print it if you wish to retain a completed copy of the form.
